Join our clients team as a Community Outreach Specialist in New York, NY, driving growth for the Program of All-Inclusive Care for the Elderly (PACE) through strategic relationship building and community education. Coordinate outreach efforts, events, and collaborate with teams to optimize referrals and enrollments. Fluency in Chinese (Mandarin and Cantonese) is required for this position.
Compensation: $23/hour
Location: New York, NY
Schedule: Monday to Friday, 8:30 AM to 5:30 PM (In-Office)
Responsibilities as the Community Outreach Specialist:

Responsibilities as the Community Outreach Specialist:
- Community Engagement: Increase awareness of PACE by educating community leaders, providers, and prospective participants about program benefits through presentations, events, and meetings.
- Relationships: Manage and maintain relationships with referral sources, senior centers, community organizations, healthcare providers, and other stakeholders.
- Coordination: Collaborate with the events and marketing teams to identify and execute community events that align with outreach strategies.
- Referrals: Track and follow up on all generated referrals, assisting with intake processes, home visits, and follow-ups as needed.
- Compliance: Ensure all outreach activities adhere to PACE guidelines and regulatory requirements, while maintaining detailed documentation and generating performance reports.
- Administrative: Develop educational materials and support the organization in marketing and public relations efforts.
- Education: Associate degree required.
- Experience: 2+ years in Healthcare Business Development required, ideally with experience in PACE, MLTC, MAP, or homecare.
- Language Requirement: Fluency in Chinese (Mandarin and Cantonese) is required.
- Skills: Strong communication, organizational, and interpersonal skills.
- Technical Skills: Proficiency in Microsoft Office (Word, Outlook), and familiarity with CRM tools preferred.
- Knowledge: Understanding of community-based long-term care services for dual-eligible and frail adults.
- Travel: Ability to travel throughout the NYC metropolitan area (80% of the time).
