Community Outreach Specialist - Four Seasons Healthcare Solutions

Duties and Responsibilities:

• Increasing patient referral flow to our agency by building relationships with physicians, long term care, independent and assisted living facilities and other community resources.
• communicate and interact with customers, clients, co-workers, and supervisors.
• Identifying, evaluating, and prioritizing potential referral sources within the agency’s territory and surrounding area.
• Market to and build relationships with a variety of referral resources to offer services across multiple patient populations.

Qualifications:

• 2-3 years sales experience
• Knowledge of physician, hospital, skilled nursing facility, case management and discharge planning service needs
• Excellent organizational, interpersonal, presentation and communication skills
• Reliable transportation, travel required
• Bi-lingual preferred.
