Position Overview:
SFAF's Syringe Access & Health Services For People Who Use Drugs is the largest syringe access and disposal health engagement program in Northern California. Our program provides evidence-based interventions to improve health outcomes for people who use drugs (PWUD) including decreasing accidental death due to overdose; reducing HIV, STI and HCV infection and transmission rates; improving engagement in care, treatment & adherence. Our mobile and stationary sites provide syringe access & disposal services, health education, counseling, linkage to substance use treatment. outreach, linkage to care and other community and health improvement-focused services.
The Community Programs Coordinator, will lead the THREADS program, a trans centered satellite syringe exchange and overdose prevention program. responsibilities include: recruitment, training, retention and supervision of community delivered syringe access and disposal, management of incentives, data collection, data entry, and performance tracking and monitoring, trainings, curriculum development, planning and facilitation, skills-building and capacity building activities.
Coordinator responsibilities include: building relationships with program participants; providing syringe access & safe disposal services; hosting lounges with programming for people who use drugs; providing overdose prevention trainings and Naloxone distribution; providing health education; facilitating groups workshops and 1 on 1 counseling; linking participants to SAS's HCV Wellness Program, HIV and HCV testing; providing referrals and linkage to substance use, health and social services; conducting street and venue-based outreach; completing all paperwork and data submissions per program protocol.
Essential Responsibilities:
- Satellite Syringe Access and Overdose Prevention Program Coordination: Coordinates the Community Satellite Syringe Exchange Programs(CSSEP). The program maintains four cohorts per year. Responsibilities include: recruitment, training, retention and supervision of the CSSEP members. Develop programming, trainings, policies and protocols, education and coaching, plan and facilitate weekly meetings, community programming curriculum, and CSSEP incentives. One of the programs is the Trans Community Satellite Syringe Exchange and Overdose Prevention Program, THREADS. This program centers the TGBN PWUD community, there are two cohorts per year.
- Community Syringe Disposal Program Coordination: Recruits people who use drugs to participate in stipend low threshold syringe clean up programming. Collect and enter syringe clean up data, manage stipends, and maintain supplies for one to two cohorts each year.
- Health Education: Develop and facilitate workshops, health education, and skills building groups. Link participants to HIV/HCV and STI testing, the HCV Wellness Program, and HIV Health Navigation team as well as out Substance Use Treatment Support Program. Provide overdose prevention, recognition and response and response training. Recruit participant volunteers for low threshold volunteer opportunities, collaborate with community program and interagency programming. Develop outreach materials, educational and resource flyers. Document and enter site data, referral data, volunteer data, Naloxone distribution data, and referrals to partner services.
- Community Engagement & outreach: Provides street and van based outreach and engagement, providing syringe access services, safe disposal. Build ongoing relationships with participants, provide harm reduction counseling and health education, referrals, and linkage to HIV/HCV testing, the HCV Wellness Program, HIV Health Navigation team. Provide on-site overdose prevention, recognition and response training along with naloxone distribution, recruit participant volunteers for low threshold volunteer opportunities, and act as an ambassador for SFAF's harm reduction programming in the community.
Other Duties:
- Shift Coverage: All Team Members schedules may include providing shift coverage for direct services and inventory activities to cover vacations, sick days, and special events. While we have scheduled hours and assigned tasks and roles in our department there there will be times when volunteer power is short, a co-worker calls in sick or is on vacation, and you will provide coverage for them.
Qualifications:
- Harm reduction focused Counseling Skills: Experience using non judgemental, strength based, trauma informed health education.
- Group Facilitation: Ability to provide group facilitation, engage group members, manage group dynamics, and meet the group's health education and/or experiential objectives. Creation and implementation of curriculum for multiple groups on various topics related to PWUD health and wellness.
- Strong understanding of harm reduction practices and principles.
- Successful Linkage and resource referral skills understanding of local substance use counseling and medication assisted treatment options, health and social services.
- Health Education: understanding of common health and mental health concerns of PWUDs, risks and harm reduction strategies to mitigate risks to health including HIV/HCV disease prevention strategies for PWUDs, vein care counseling, safer smoking, overdose response and prevention, substance use management , and safety planning.
- De-escalation, conflict resolution, and crisis interventions. Exhibits sound judgment and skill in navigating and de-escalating client conflicts and crisis.
- Driving: Proof of valid California driver's license with excellent driving record required.
- Foster Teamwork: Work cooperatively and effectively with others to set goals, resolve problems, and make decisions that enhance program effectiveness.
- Build Relationships and Partnerships: Ability to engage and maintain positive working relationships with others, both internally and externally, to achieve the goals of the program and the organization.
- Flexibility: All department positions require staff to offer flexibility to meet the needs of the team and handle unexpected changes in their workday or workweek.
Physical Requirements:
Skill in operating office equipment such as a personal computer, copy machine and telephone system. Ability to perform routine bending, stooping, twisting, and reaching. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Ability to lift a minimum of 50 pounds. The position may have exposure to blood borne pathogens and other infectious agents.
Education and Experience:
- Associates Degree preferred.
- Minimum 3 years' experience working with people who use drugs, highly marginalized, or people experiencing homelessness required.
- 2 years of experience with direct health education for PWUD required.
- People with lived experience relevant to the role strongly encouraged to apply.

Work Environment:
This position is primarily located in the field and at 117 6th Street; San Francisco, however may require traveling to other office locations or off-site meetings as required by the job
