Job Posting: Certified Community Health Worker

Location: Remote (specific counties in Florida: Orange, Hillsborough, Polk, Osceola)
Salary Band: $55,000 annually

Position Overview:

We are seeking a passionate Certified Community Health Worker to enhance our outreach efforts and address social determinants of health within Florida communities. This role focuses on direct member and provider engagement, health risk assessments, and building partnerships with community-based organizations. Key Responsibilities:

• Conduct member and provider outreach (both telephonic and face-to-face)
• Complete health risk assessments and screenings
• Serve as the health plan liaison to community-based organizations
• Document outreach efforts, successes, and barriers using our clinical documentation system
• Address social barriers through closed-loop referrals
• Report on outcomes related to outreach and program success

Performance Expectations:

• Maintain compliance with member and provider outreach
• Participate in quality case review audits
• Meet defined key performance metrics for pilot initiatives (e.g., reducing unreachable populations, decreasing ER visits)

Qualifications:

Certified Community Health Worker Required Skills:

• Community Health Worker Certification (mandatory for Florida applicants)
• Relevant experience in case management, community engagement, care planning, and assessment completion
• Knowledge of social determinants of health (SDOH) barriers
• Familiarity with community organizations in Orange, Hillsborough, Polk, and Osceola counties
• Valid driver’s license

Certified Community Health Worker Preferred Skills:

• Experience in roles such as Community Health Worker, Peer Support Specialist, or Housing/Employment Specialist
• Proficiency in Microsoft Office Suite (Word, Outlook, Excel)

Certified Community Health Worker Interview Process:

• Interviews will be conducted via web conference, and one interview is expected (excluding Talent Acquisition screening).

Certified Community Health Worker Work Environment:

This position is remote, focusing exclusively on the specified counties in Florida (Orange, Hillsborough, Polk, Osceola). Preference will be given to candidates residing in these counties due to mileage reimbursement policies. While candidates from nearby counties like Pasco, Lake, Manatee, Brevard, and Hardee may apply, the ideal candidate will be from one of the specified counties.
