Metropolitan Human Services District (MHSD) is one of ten Local Governing Entities (LGE) serving persons with Mental Health Disorders, Addictive Disorders and Intellectual/Developmental Disabilities in Louisiana. Legislated by the State and through its Board of Directors, MHSD is responsible for the operation and management of community-based programs and services for the parishes of Orleans, St. Bernard, and Plaquemines.
We are currently hiring for a Community Service Professional 3.
This is a high position within the Metropolitan Human Services District (MHSD) DDS office having responsibility for providing support coordination services for individuals who have met criteria for participation in the DD service system. The incumbent plans and coordinates a portion of a statewide program element in the geographic service area. This position involves the knowledge of(MR/DD) Law (LRS28:380), Act 659) and knowledge ofa complex system of state and federally funded programs including the Title XIX Medicaid Waiver Program and Medicaid Case Management Provider Agencies. Utilization of independent and in-depth knowledge of policies, procedures, and governmental and community resources and technical expertise in dealing with a variety of systems and professionals

Minimum Qualifications
MINIMUM QUALIFICATIONS:
A baccalaureate degree plus three years of professional level experience in developmentally disabled services, alcohol/drug abuse counseling or treatment, mental health treatment, health care management, or social services.
SUBSTITUTIONS:
Six years of full-time work experience in any field may be substituted for the required baccalaureate degree.
Candidates without a baccalaureate degree may combine work experience and college credit to substitute for the baccalaureate degree as follows:
A maximum of 120 semester hours may be combined with experience to substitute for the baccalaureate degree.
30 to 59 semester hours credit will substitute for one year of experience towards the baccalaureate degree.
60 to 89 semester hours credit will substitute for two years of experience towards the baccalaureate degree.
90 to 119 semester hours credit will substitute for three years of experience towards the baccalaureate degree.
120 or more semester hours credit will substitute for four years of experience towards the baccalaureate degree.
College credit earned without obtaining a baccalaureate degree may be substituted for a maximum of four years full-time work experience towards the baccalaureate degree. Set up and maintain case records on individuals assigned who are referrals for the Developmental Disability support and services system.
Schedule an interview with clients and families for either a face-to-face interview in the office or in the community to initiate the criteria for participation into the OCDD Service System including explanation of the purpose and intent of the program and services.
Discuss and request information from client and/or family member information to document developmental disability and the importance of the face-to-face interview date.
Assist client and families in completing the required forms and if necessary, obtain signed consent to release information and the Request for Participation forms.
Determines the current status of individual to evaluate for immediate or at risk is present.
Adult individuals applying with the System Entry process are to be offered the opportunity to register to vote by completing the Voter Declination form to determine if requesting to register to vote or not to register to vote. The Voter Registration form to be completed and mailed to the Registrar of Voters office if requesting to register to vote or to change address.

Receives Early Steps referrals once parent/guardian responds to request for System Entry process from OCDD Early Steps Regional Coordinator and/or Early Steps support coordinators on infants and toddlers with disabilities who are transitioning form the Early Steps (early intervention) program. To process in accordance with the System Entry policy and procedure and ensure required document with the Early Step "protected date" form is included with the referral packets.
Enters requires information into the Participant Services Database System for completion of the initial System Entry process decision determining for an individual and on the redetermination process decision for an individual.
Review and validate monthly reports and information on individuals' application status and to insure in compliance with the System Entry compliance dates for completion.
15 % - DDS Intake Process:
Receive intake calls from individuals or referrals on rotating schedule basis who are applying for participation into the developmental disability services system as part of the weekly on-call rotation for the entry unit or as scheduled.
Completes MHSD-IDD Application Request form and inputs data into OCDD Database and IDD Records.
Prepares and responsible for formatting of individual's record in accordance with the OCDD Central Record process.
Coordinate with client and family member of the scheduled interview date and ensure required notices are sent to the client and family member including what documents to bring to the interview what will the interview consist of and information about MHSD-DDS supports and services.
Work with the Database Administrator by ensuring that individuals' information is input accurately into the database system.
Schedule eligibility redeterminations/initial eligibility determination appointments for Persons Served. These duties include:
Completing the Eligibility Initial Contact Form with the PS
Scheduling the next available or PS preferred appointment date and time utilizing the Entry Calendar on Microsoft Outlook.
Explaining the required documents needed for the Eligibility appointment utilizing the Eligibility Intake Script.
Mailing and/or emailing the required Eligibility forms to the PS/family.
Adding the Person Served to the OCDD Database System (if applicable) and updating all required elements (demographics, caregiver/contact).
Updating the OCDD Database System Entry Application and Case Management section indicating the Community Service Professional (CSP) assigned.
Compose the Confirmation of Appointment Letter or the Notice of Redetermination Letter and mail/email to the PS/family.
Adding the Person Served to the electronic IDD database (if applicable)
Enter all activity elements in the electronic IDD record.
15 % PASRR- Pre-Admission Screening and Annual Resident Review
Complete Pre-Admission Screening and Annual Resident Review (PASRR) Evaluation per the DHH policy to determine if Nursing Home placement is recommended.
Ensure that PASRR evaluations are completed and submitted to supervisor within seven (7) working days upon receipt of request. Notify supervisor if the timelines cannot be adhered to due to extenuating circumstances.
Resubmit the PASRR evaluation to the Supervisor once all corrections have been completed for review. Once reviewed and approved submitted to Office of Aging and Adult Services (OAAS) and the Nursing Homes.
10 % - Entry Review Team Process
Inform Supervisor of the need to present individuals for an Entry Review Team meeting.
Meet with the Entry Review Team to present the Individual Entry Review report and documents available to make determination for decision on whether the individual meets criteria for participation or does not meet criteria for participation in the Developmental Disability services systems.
Follow up with the Entry Review Team recommendation and obtain required information on the Individual Entry Review form for the individual's record.
5% - Appeal Process: Prepare packet of summary and documents
Assist in preparation of packet of summary and documents once appeal notification is received from the Division of Administrative Law - health and Hospitals Section.
Attends administrative appeal hearings to represent MHSD-DDS office. Ensure that the completed appeal packet and documents are submitted to the individual, family and to the Administrative Law Judge before the scheduled appeal hearing date.
5 % - Other Duties As assigned by the Director to ensure the efficient operations of the MHSD- Developmental Disabilities Office. Assists with other units by inputting information into the database system as needed. Attends supervisory and staff meeting as scheduled or requested.

Work with the Database Administrator by ensuring that individuals' information is input accurately into the database system.
Schedule eligibility redeterminations/initial eligibility determination appointments for Persons Served. These duties include:
Completing the Eligibility Initial Contact Form with the PS
Scheduling the next available or PS preferred appointment date and time utilizing the Entry Calendar on Microsoft Outlook.
Explaining the required documents needed for the Eligibility appointment utilizing the Eligibility Intake Script.
Mailing and/or emailing the required Eligibility forms to the PS/family.
Adding the Person Served to the OCDD Database System (if applicable) and updating all required elements (demographics, caregiver/contact).
Updating the OCDD Database System Entry Application and Case Management section indicating the Community Service Professional (CSP) assigned.
Compose the Confirmation of Appointment Letter or the Notice of Redetermination Letter and mail/email to the PS/family.
Adding the Person Served to the electronic IDD database (if applicable)
Enter all activity elements in the electronic IDD record.
15 % PASRR- Pre-Admission Screening and Annual Resident Review
Complete Pre-Admission Screening and Annual Resident Review (PASRR) Evaluation per the DHH policy to determine if Nursing Home placement is recommended.
Ensure that PASRR evaluations are completed and submitted to supervisor within seven (7) working days upon receipt of request. Notify supervisor if the timelines cannot be adhered to due to extenuating circumstances.
Resubmit the PASRR evaluation to the Supervisor once all corrections have been completed for review. Once reviewed and approved submitted to Office of Aging and Adult Services (OAAS) and the Nursing Homes.
10 % - Entry Review Team Process
Inform Supervisor of the need to present individuals for an Entry Review Team meeting.
Meet with the Entry Review Team to present the Individual Entry Review report and documents available to make determination for decision on whether the individual meets criteria for participation or does not meet criteria for participation in the Developmental Disability services systems.
Follow up with the Entry Review Team recommendation and obtain required information on the Individual Entry Review form for the individual's record.
5% - Appeal Process: Prepare packet of summary and documents
Assist in preparation of packet of summary and documents once appeal notification is received from the Division of Administrative Law - health and Hospitals Section.
Attends administrative appeal hearings to represent MHSD-DDS office. Ensure that the completed appeal packet and documents are submitted to the individual, family and to the Administrative Law Judge before the scheduled appeal hearing date.
5 % - Other Duties As assigned by the Director to ensure the efficient operations of the MHSD- Developmental Disabilities Office. Assists with other units by inputting information into the database system as needed. Attends supervisory and staff meeting as scheduled or requested.
