Are you passionate about helping others and building stronger communities? We’re seeking a Community Support Coordinator to provide essential services and connect residents with the resources they need to thrive. This role involves offering short-term case management, conducting assessments, and facilitating outreach efforts to promote stability, independence, and a safe living environment within our housing communities.
Key Responsibilities:
- Provide direct support to residents through short-term case management and resource navigation.
- Build and maintain a network of referrals for social services and community resources.
- Manage and update databases for tracking resident needs, program participation, and communication.
- Plan and coordinate workshops, social activities, and community events.
- Conduct research and contribute to the development of special programs and resource initiatives.
- Support the development of policies and procedures for community programs.
- Identify and implement effective practices for case management and program operations.
Education and Experience:
- 1 or more years of college coursework in social services or a related field.
- At least two (2) years of experience in social services, case management, or a related area.
- Bilingual (English/Spanish)
