Consumer Advocate/Behavioral Health - Old Colony Elder Services

OCES supports elders and individuals with disabilities by providing vital information and coordination of services. By promoting healthy, save lives for our consumers, we hope to allow them to stay as active and engaged in their lives and in their own communities for as long as possible.

To that end we are seeking a Consumer Advocate to provide support to OCES consumers in accepting and retaining home care program services.

What you will do:

• Work within an Interdisciplinary Team, consulting with Home Care, Protective Services, and others as appropriate
• Conduct in-home visits as necessary to evaluate environment, functional capacity, and resources of referred consumers
• Develop appropriate goal-oriented intensive care plans
• Assist in arranging for behavioral health services as necessary
• Provide comprehensive documentation of all consumer-related interactions
• Complete statistical data as required by agency

What you will bring:

• Bachelor's degree in social work or related field, Master's preferred
• Social Work License or LMHC preferred.
• Minimum of two years' experience in behavioral health or case management
• Considerable knowledge of the social, emotional, and physical processes of aging
• Knowledge of at-risk behaviors and mitigation strategies
• Ability to assess risk and respond appropriately to emergency situations.
• Ability to hold the confidence of consumers, caregivers, and their families.
