Responsibilities
Provide coordination of patient care throughout the healthcare continuum.
Responsibilities:
+ Provide coordination of communications with patients, families, physicians, and healthcare team members.
+ Coordinate patient scheduling with appropriate resources or clinical services and maintains required documentation.
+ Support standard patient access initiatives across the healthcare continuum.
+ Provide support with navigating the healthcare system and assists patients with utilizing available community support services.
Requirements
Licensure / Certification / Registration:
One or more of the following preferred:
+ Licensed Social Worker credentialed from the Indiana Behavioral Health and Human Services Licensing Board.
+ Licensed Social Worker specializing in Clinical Practice credentialed from the Indiana Behavioral Health and Human Services Licensing Board.
Education:
+ High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.
Additional Preferences
Mammography Tracking experience required. RT(R)(M) current or former.
