Contract Host Home Provider - Palmers Home Care

Job Overview:
Palmer’s Home Care is seeking dedicated Host Home Providers/Live-In Staff to create a safe, supportive, and therapeutic environment for individuals with disabilities. This role involves fostering personal growth, rehabilitation, and community integration for those you support, while working closely with relief staff to ensure high-quality care is consistently provided.

Key Responsibilities:

• Provide 24/7 care in a clean, safe, and healthy living environment.
• Assist individuals with transitions, move-ins, and move-outs.
• Actively participate in Person-Centered Planning Teams and Individual Support Plan (ISP) meetings.
• Attend to the individual’s physical health, emotional well-being, and support their involvement in family and community life.
• Facilitate access to community services, including recreational and religious activities, shopping, and volunteering.
• Maintain professional documentation of services, including progress toward goals outlined in the ISP.
• Follow medication administration policies and notify of any changes.
• Provide nutritious meals and snacks, as well as transportation to appointments and activities.
• Report any critical events to the agency promptly.
• Maintain required insurance, including homeowner’s/renter’s and vehicle insurance.
• Protect the confidentiality of all individual-related information.
• Communicate openly with Palmer’s Home Care, Support Coordinators, and the individual’s family/guardians.

Qualifications:

• Ability to provide care on a 24-hour basis.
• Strong communication skills and commitment to maintaining a safe, supportive environment.
• Must comply with Division of Developmental Disabilities (CMRO/DMH) training requirements.
• Valid driver's license, properly registered and insured vehicle.
• Homeowner’s or renter’s insurance.
