Job Duties:
· Coordinate nutritional care of residents and patients by completing nutritional assessments.
· Participate in care planning meetings as required.
· Provide nutritional counseling and education for residents and family.
· Completes comprehensive assessments on patients as required.
· Provides dietary in-service training as required.
· Under PDPM, identify, document, and treat nutrition related diseases for improved outcomes.
· Meet DOH/state/federal and company standards.
· Perform duties independently without training or direction.
· Perform other duties as assigned by management.
Required Skills/Abilities:
· Good decision making and judgment capabilities.
· Ability to identify and solve complex problems.
· Great communication and interpersonal skills are a must.
· High comfort level with enteral nutrition, wounds, weight management, Hemodialysis.
· Comfortable working in a healthcare facility.
· Familiarity with state survey process and compliance with nutrition care regulations.
Education and Experience:
· Bachelor’s degree in nutrition, health, or related field.
· Must possess Registered Dietitian licensure within the state of operations
· At least one year of experience in a long-term care setting
· Experience with MDS 3.0.
· Experience with electronic charting such as Point Click Care is a plus
Physical Requirements:
· Prolonged periods standing and walking.
· Must be able to lift up to 25 pounds at times.
