Center for Life Management (CLM) provides Southern New Hampshire's most comprehensive array of outpatient mental healthcare. The Coordinated Entry Specialist provides support to ensure that all persons in Western Rockingham within the Balance of State Continuum of Care (NH-500), who are at risk of or experiencing homelessness to gain access to appropriate shelter/connections and/or to permanent housing and other necessary services through referrals, assistance, supportive services, intervention, diversion and prevention. Towns included in Western Rockingham County: Atkinson, Chester, Danville, Derry, Hampstead, Newton, Pelham, Plaistow, Salem, Sandown, and Windham. On occasion Londonderry and possible surrounding towns. This position is grant based and may end by/around the date of June 30th, 2025. This is a full time, 40 hour per week position.
Major Functional Areas and Description of Duties
- Assists those experiencing or imminent risk of homelessness in identifying immediate needs;
- Will utilize housing problem solving, and prevention and diversion strategies, as appropriate;
- Completes the necessary housing assessments as required by the Balance of State Coordinated Entry System with eligible households;
- Honors participant choice in making and taking referrals;
- Provide or arrange for provision of housing-focused case management and voluntary supportive services;
- Maintains relationships with community providers in order to facilitate referrals to community providers as needed;
- Assists households in gathering and completing the necessary documentation in order to obtain permanent housing;
- Gathers data and reports availability of homeless services to include providers not receiving funding from the state;
- Responsible for keeping up accurate intake/service records within HMIS according to the requirements of the Bureau of Homeless Services (BHS) HMIS requirements and HUD’s data standards;
- Knowledge of community resources, particularly housing and homelessness resources;
- Responsible for having adequate knowledge of all CLM programs and will gather sufficient intake information to make referrals to other CLM programs which are beneficial to the client and his/her family;
- Ability to solve problems, make decisions, deal calmly in crisis situations, resolve conflicts and engage in active listening;
- Assist, guide, and support the PATH Outreach staff covering this geographic area;
- Collaborates with PATH Outreach Worker daily, as well as but not limited to wearing comfortable clothing and appropriate footwear with walking through many different types of terrain throughout Western Rockingham County with PATH Outreach Worker;
- Serve on appropriate committees, both internal and external, as determined by supervisor;
- Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients and staff;
- Execute all required services and timely documentation on each service delivered in accordance with agency policies and federal/state standards;
- Maintain professional memberships, licenses, accreditation, certifications, etc.;
- Represent CLM in a professional and appropriate manner in all settings;
- Perform all other duties as assigned by supervisor;
Qualifications and Skills
- Associates or Bachelor’s degree in related field, but not required;
- Experience with community-based initiatives and knowledge of poverty-related needs, issues and service delivery systems, with a passion for assisting disadvantaged persons in accordance with the mission of Community Action;
- Strong communication, interpersonal, and customer service skills required to interact with clients, families, staff and visitors in a professional manner;
- Ability to work in a fast-paced environment with strong organizational skills, ability to problem solve, multi-task, and manage competing tasks while attending to details and adhering to deadlines;
- Ability to maintain composure and exercise sound judgment in routine, urgent, and emergent administrative tasks;
- Proficient computer skills and knowledge of Microsoft office applications with emphasis on Word, Excel, Outlook and TEAMS;
- Ability and desire to work as part of a team while also able to be self-directed and motivated with minimal supervision;
- Flexibility to adapt schedule and fill in as needed;
- Ability to acquire and effectively execute new skills as needed according to evolving department needs;
Physical Demands and Working Conditions
The majority of the work takes in an office setting with occasional remote work permitted. Manual dexterity is needed to use computers and standard office equipment. The ability to sit or stand for extended periods of time is required. Reliable transportation is required for travel to/from/with clients and between office locations. A clean driving record, valid driver’s license, proof of auto insurance coverage and reliable internet for remote work is required for this role.
