Who you are: a motivated individual with a Masters in Social Work who meets the requirements of Social Worker as stated in the Public Health Code, knowledgeable in Admissions and discharge procedures within skilled nursing facilities in conjunction with all current laws. Familiar with residents' rights and documentation requirements in accordance with O.B.R.A regulations and/or guidelines. A professional with multiple years of long term care experience.

Job Description:
The Corporate Director of Social Services is based out of the corporate office, this position will be mainly on the road, traveling between our facilities. The Corporate Director of Social Services plans, controls, organizes and executes the admission and discharge process in accordance with current federal and state regulations and guidelines. Develops and maintains relationships with referral sources, residents, and family members in order to promote the overall psychological, social and emotional well-being of each resident ensuring the highest quality of care is given while fostering Quality Improvement methods and striving to attain the company’s mission statement. Provides support and guidance to Social Services Designees at the facility level.
Required Qualifications:
- At least two years experience in a Social Services capacity within long-term care
- Licensed Clinical Social Worker in Connecticut in good standing
- Deep working knowledge of the admissions and discharge procedures within long-term care
- Master of Social Work degree
