Description
SUMMARY: Under the supervision of the Program Director, provides clinical services to patients.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties and responsibilities may be assigned.
+ Conducts assessments and provides individual counseling and treatment/recovery plan preparation, monitoring, and review.
+ Provide therapeutic interactions to foster the patient’s social, cognitive, and behavioral skill development.
+ Provide psychosocial interventions to teach skills for coping with urges, craving, impulsive behavior and cognitive distortions in thinking, motivational interviewing techniques.
+ Plan and facilitate interactions with residents within the milieu to build social, emotional, and behavioral functioning.
+ Communicate and collaborate with legal entities on referrals, treatment, progress, etc.
+ Prepare and maintain case records.
+ Participate in weekly staff meetings.
+ Attend weekly supervision.

Qualifications
REQUIRED EDUCATION
+ High School Diploma or equivalent
+ Master's degree in Social Work
LICENSE/CERTIFICATION
+ LMSW
REQUIRED EXPERIENCE
+ Minimum two years in a behavioral health/substance use disorder treatment program
WORK ENVIRONMENT
+ This job operates in an indoor office environment. This role routinely uses standard office equipment such as computers, phones, and photocopiers.
+ This position is hybrid/mobile and will be working between Suffolk County Jails, out in the community, as well as in an office at a Phoenix House facility.
