Overview
Credence Management Solutions, LLC, is seeking a DHA Patient Safety Consultant (DHA PSC) to assist and advise the Government with the establishment and operation of the Defense Health Agency Patient Safety Program (DHA PSP).
This position is remote.

- Provide oversight and support to the Military Treatment Facility (MTF) Patient Safety Program Points of Contact (POCs) for the establishment and operation of the DHA PSP. Stay abreast of national and DoD PS initiatives and their impact on the DHA PSP.
- Assist the MTFs on their organization-wide processes to reduce the risks of preventable medical events by conducting Proactive Risk Assessment (PRA) and Performance Improvement (PI) activities.
- DHA PSCs must have experience with and comply with Department of Defense (DoD)-mandated patient reporting systems such as TapRooT and PSR and provide consultation on how the MTFs uses these methods to identify system and process issues that could have or did result in harm. Help clarify the classification of each reported event with the MTF PSP POCs and determine the level of assessment required.
- Coordinate with the MTF PSP POCs to ensure they complete facility-wide implementation of healthcare team training through various DoD training venues such as Train-the-Trainer, Train-the-Staff On-site, and Awareness. After initial team training, individuals will complete workplace-based sustainment training annually.
- Coordinate with the MTF PSP POCs to ensure they conduct Comprehensive Systematic Analysis (CSA) such as a Root Cause Analysis (RCA) on PS events to identify causal factors and assist with establishing action items to correct and/or improve processes and systems to eliminate or decrease the risk of event reoccurrence. Upon determination that a CSA shall be performed, the contractor personnel shall monitor progress to ensure compliance with all deadlines.
- The product of the CSA is the action plan. PSRCs will verify that the MTF PSP POCs complete an action plan within 45 calendar days from the date of the event. The action plan identifies actions that the organization intends to implement to reduce the risk of similar events occurring in the future and that it includes responsibility for each action item, timelines, and a plan for measuring the effectiveness of the actions. Follow-up and monitoring of completed CSA actions will be performed for two years after the event to ensure sustainment of implemented actions.
- DHA PSC shall verify that the MTF/ PSP POCs have briefed the commander and senior leadership upon completion of the CSA and obtain written approval for the CSA and action items at that time.
- Verify the MTF/ PSP POCs establish a mechanism to track completion, effectiveness, and sustainment of action items resulting from CSA and analysis of near miss and actual events not requiring an CSA.
- Participate in a cell review of all CSAs and subsequently provide constructive feedback to the MTF Chief of Medical Staff (SGH), MTF PSP POC, and any other members of the MTF.
- Education and Training Activities. DHA PSC shall assist with the establishment and execution of an enterprise-wide PS education program, verifying MTF-level PSP POCs execute required training elements within prescribed timeframes.
Education, Requirements and Qualifications
- Public Trust Investigation required. An active Secret clearance or higher preferred.
- A Master's degree in nursing, health care administration or an equivalent degree.
- A minimum of fifteen (15) years of experience at the healthcare facility level is required. Military experience and/or experience in an MTF are preferable..
- Proficient skills in Microsoft Suite of tools, including Outlook, Word, Excel, and PowerPoint is required. Awareness of Microsoft Access, Visual Basic, SQL, relational databases, visual report design tools. Experience with quantitative and qualitative data analysis.
- Full time experience working in Patient Safety.
- The ability to work collaboratively with risk management and quality.
- The ability to organize time and manage multiple projects simultaneously.
- Knowledge of The Joint Commission (TJC) and/or Accreditation Association for Ambulatory Health Care (AAAHC) standards and scoring guidelines.
- One year experience with performing RCAs, PRAs and/or PI at a healthcare facility level plus a minimum of two (2) years of program management experience.
- Expert skills in written and oral communications, public speaking and interpersonal relationships in order to participate in multidisciplinary forums, such as: DoD committees, working groups, workshops, orientation and education programs for medical and non-medical staff. Prepares aggregate performance reports and project status presentations, assists or make recommendations in the development of operating guidance, and briefs DoD leadership as appropriate
