Diabetes Patient Education and Care Coordination Provides high quality education, training, and care coordination for patients with diabetes in the acute care setting.Oversees the planning, implementation, and evaluation of diabetes education.Ensures diabetes education handouts/videos are aligned with national standards of care.Maintains diabetes website accessible through organization intranet.Develops, implements, and coordinates education programs to successfully aid learners in achieving pre-established measurable outcomes.Uses various teaching strategies and adult learning principles appropriate to the environment, person, or group being instructed. Diabetes Program Oversight Identifies patient and organizational education needs by performing gap analysis. Uses data to develop strategy to address the performance gap.Evaluates outcomes and collects data for program revision/initiation.Evaluates effectiveness of targeted education programs for desired behavior change.Oversees diabetes related policies and procedures and ensures alignment with best practice.Fulfills a lead role in committees specific to diabetes care, including collaboration with representatives for patient education and the electronic medical record.Collaborates with Laboratory Point of Care Testing and Pharmacy representatives to ensure optimum workflows for diabetes care.Provides diabetes education to clinicians upon hire and ongoing based on gap analysis. Consultation Improves staff performance by consulting with operating departments.Uses expertise to collaborate with clinicians to improve performance regarding diabetes care.Assesses individual and team performance (performance analysis) Identifies factors impeding and contributing to performance (cause analysis).Selects, recommends, and evaluates interventions (interventions).Facilitates the implementation of interventions and the adoption of new behaviors (implementation).Facilitates the development and use of measures and evaluation strategies (evaluation).Communication and Relationships Develops collaborative relationships through effective communication skills; Consistently uses: assertive communication, active listening skills, positive and constructive feedback, and conflict resolution models.Effectively articulates department/organization vision and goals.Acts as role model, coach, mentor, and resource person to individuals by using advanced communication skills.Fosters interdisciplinary and interdepartmental collaboration.Uses appropriate avenues and methods of communication when developing and implementing proposals, plans, and programs. LEVEL OF EDUCATION Minimum: Bachelor's degree.
Preferred: Master's degree.
CERTIFICATIONS, LICENSES, REGISTRATIONS
Minimum: Registered Dietitian credential.
American Heart Association Basic Life Support certification.
Certified Diabetes Care and Education Specialist (CDCES).
TECHNICAL REQUIREMENTS
Minimum: Outlook, PowerPoint, Clinical EMR experience.
Preferred: Experience in clinical practice with insulin pump.
YEARS OF RELATED WORK EXPERIENCE
Minimum: Five (5) years professional experience in a clinical setting related to diabetes education. Knowledge of adult education theories, principles, and practices.
Preferred: Academic and/or experiential preparation in program management.
