A Dietitian provides nutritional counseling to the patient and/or primary caregivers in order to address nutritional issues related to the patient's health or disease state within the context of the overall plan of care. Utilizes assessments, diagnostic tools, and food service systems to maintain high quality patient care and therapeutic services.
Essential Duties and Responsibilities Dietitian:
- Provides skilled nutrition services to patients and families in accordance with the policies and procedures of Hospice Buffalo, Inc. and Hospice Inpatient Unit, as well as state and federal guidelines to meet needs.
- Participates in Interdisciplinary Group/Team (IDG) meetings when possible, providing active incorporation of the nutrition needs of the patient within the plan of care and updates the plan of care when appropriate.
- Demonstrates knowledge of cultural and ethnic diversity in end of life care and conveys this to staff.
- Assesses and reassesses nutrition factors in order to help the patient and family cope with problems in daily living and to help them follow medical recommendations to maximize patient's quality of life.
- Instructs and advises patients, family, and other IDG members in the principles of good nutrition in which they may assist the patient. Instructs all of the above in food selection and preparation alternatives and supplements to enhance the patient's quality of life.
- Assists the physician and other members of the hospice team to understand the significance of nutrition and emotional factors related to the client's case.
- Participates in patient conferences as appropriate, demonstrating sound management of issues presented.
- Demonstrates appropriate infection control procedures to include sharps disposal/hand hygiene, waste disposal, and use of personal protective equipment.
- Adheres to requirements of both Hospice & Palliative Care Buffalo, Inc. and Hospice Buffalo, Inc. policies and procedures that are both patient focused (clinical) and organizational focused (administrative). Adheres to The Health Insurance Portability and Accountability Act (HIPPA) regulations, policies and standards of confidentiality.
- Collaborates with other agencies and professionals to facilitate patient/family care in a timely fashion.
- Demonstrates ability to perform a comprehensive nutrition therapy assessment, identifying the nutrition related needs of the patient, safety issues, and documents on the initial assessment.
- Develops an individualized patient/family plan of care plan which reflects changes as appropriate.
- Effectively executes patient/family/caregiver/facility staff teaching in all given situations.
- Appropriately explains duties and functions of the interdisciplinary team.
- Elicit no founded patient/family complaints or unusual occurrences related to standards of care.
- Documents to agency standards to include: Completion of signatures and documentation of care in a timely manner using A-I-R protocol to support hospice stay in program and level of care.
Qualifications for a Dietitian:
- Education/Certification
- Bachelor's degree program in food and nutrition from a college or university approved by the education department that is also registered by the American Dietetic Association or the equivalent of such training, experience, and active participation in continuing dietetic education.
- Registration and membership in the American Dietetic Association (ADA) preferred.
- Valid Driver's License, free of serious violations, and vehicle required for use when on duty.
- Experience
- Minimum of 2 years of experience required; Hospice and/or home care experience preferred.
