$18.25 an hour plus 16 hours of PTO once med certification is completed
**Job Summary:**
The Direct Care Coordinator will provide habilitative assistance and care to clients in various Agency programs, including community and supported living arrangements, family support services program, and day/vocational program. Responsibilities include direct daily care, teaching, supervising clients, and maintaining accurate records.
**Responsibilities:**
- Supervise clients, maintain awareness of their status and location at all times.
- Assist in developing and implementing Individualized Plans of Service or Individual Educational Plans for each client.
- Teach, supervise, and assist clients in daily living activities such as personal care, hygiene, shopping, and community participation.
- Monitor and report client progress on goals, and ensure client rights are protected.
- Complete incident reports, medication error reports, and implement behavioral plans when necessary.
- Implement therapist recommendations and programs, develop recreation and community activities.
- Collaborate with Clinical Director, Consultants, families, schools, and other agencies.
- Maintain accurate record-keeping of services provided to clients and specific goals.
- Attend staff meetings, in-services, trainings, and maintain required certifications.
- Administer medications, provide routine health care, first aid, and transportation for clients.
- Ensure cleanliness and organization of program sites and equipment.
- Conduct and participate in emergency drills as required.
- Perform any other related duties as assigned.
**Qualifications:**
- Successful completion of Medication Certification Program.
- Completion of PMT, CPR, and First Aid training.
- Valid CT driver's license with a satisfactory driving record.
**Education and Experience:**
- High School Diploma or equivalent.
- Minimum of two years of experience working with individuals with developmental and/or behavioral challenges preferred.
**Physical Demands:**
- Ability to lift and move up to 75 pounds, exert force when needed for physical restraint procedures.
- Vision abilities to adjust focus, and perform tasks requiring close vision.
- Regular activities include standing, walking, sitting, and using hands.
**Skills:**
- Ability to read and interpret documents in English, write reports, and correspond effectively.
- Proficiency in mathematical skills for basic calculations.
- Strong reasoning ability to follow instructions and solve problems with multiple variables.
