Job Summary:
The Direct Care Professional will provide habilitative assistance and care to clients in various Agency programs. This includes community and supported living arrangements, family support services, and day/vocational programs. The main responsibilities of this role are to provide direct daily care, teaching, and supervision of clients, as well as record-keeping and maintaining awareness and safety of the individuals.
Responsibilities:
- Directly supervise clients, ensuring knowledge of their status and location at all times.
- Assist in the preparation and implementation of individual client plans.
- Teach, supervise, and assist clients with adult daily living activities such as personal care, hygiene, meal-planning, shopping, and community participation.
- Monitor client progress on goals and report incidents of abuse, neglect, and mistreatment.
- Complete incident reports and medication error reports when necessary.
- Implement behavioral plans and approved or emergency restraints and procedures.
- Implement recommendations and programs from occupational, physical, and speech therapists.
- Develop and implement recreation and community participation activities for clients.
- Collaborate with Clinical Director, Consultants, families, schools, and other agencies in goal implementation.
- Maintain accurate and timely record keeping of client services and goals.
- Work cooperatively with supervisory staff, other Agency staff, and external agencies.
- Attend required staff meetings, in-services, and trainings.
- Administer medications, provide routine health care, and first aid to clients.
- Maintain cleanliness and organization of program sites and transportation vehicles.
- Transport clients to and from activities and programs.
- Conduct and participate in emergency drills as required.
- Complete and maintain Medication Certification program.
- Ensure accurate record keeping of petty cash and client cash on hand accounts.
- Perform other related duties as assigned by the Program Director or Assistant.
Qualifications:
- Successful completion of Medication Certification Program.
- Successful completion of PMT, CPR, and First Aid training.
- Valid CT driver's license with a satisfactory driving record.
- High School Diploma or equivalent.
- Minimum of two years of experience working with individuals with developmental and/or behavioral challenges preferred, or equivalent combination of education and experience.
Physical Demands:
- Ability to lift and move up to 75 pounds and exert over 100 lbs. of force for physical restraint procedures.
- Ability to stand, walk, sit, use hands to handle objects, reach with hands and arms, talk, and hear.
- Visual abilities required include close vision and the ability to adjust focus.
Language Skills:
- Ability to read and interpret English documents and write routine reports and correspondence in English.
Mathematical Skills:
- Ability to perform basic mathematical calculations.
Reasoning Ability:
- Ability to apply common sense understanding to carry out detailed instructions and solve practical problems.
Work Environment:
The work environment is usually moderate in noise level, and the employee may be exposed to changing weather conditions while traveling by automobile.
This position is Full Time and is located on Allernton Rd, Naugatuck, CT.
