About the Job
Role: To provide quality living and community support and be responsive to individuals' needs in residential, work or community settings. Teach, support and facilitate choices for individuals in their homes, work or community settings.
Major Duties and Responsibilities:
- Implement Person Centered Support Plans for assigned individuals.
- Assist with daily living skills including, but not limited to, meal preparation, general hygiene, toileting, general housekeeping, work and leisure activities.
- Administer medication and treatment(s).
- Complete documentation as required through the general course of duties.
- Report to the appropriate Program Coordinator, Program Manager, Case Manager or Wellness Liaison any occurrences that affect the immediate well-being of the individual.
- Attend all meetings as assigned and required.
- If approved as a driver of agency vehicles, employee may transport individuals.
- Other duties as assigned.

- Adhere to scheduled hours.
