Primary responsibilities:
This position is responsible for ensuring that all primary responsibilities are delivered in compliance with
the company’s policies and procedures, Joint Commission standards (TJC), Licensing and Regulatory
Affairs (LARA), HCBS guidelines and the contractual requirements of Community Mental Health Agencies
and other regulatory bodies. As a Direct Support Professional, you are expected to be aligned with the
goals and mission of the organization, and to perform in accordance with our “I CARE” core values of the
organization – Integrity, Compassion, Advocacy, Respect and Excellence.
Responsibilities/Essential Functions:
Sets positive examples for residents at all times.
Adheres to the person-centered plans, and behavior plans as written for each resident.
Provides opportunities throughout the day to help residents meet their goals. (An example would
include teaching groups to follow activity programs and encouraging resident attendance.)
Provides quality of care and a safe and healthy setting free from harm while upholding the rights
of all residents. Creates a space of trust and relationship with all residents.
Protects all residents from abuse and/or neglect and immediately reports any occurrence to
appropriate entities.
Maintains and protects the confidentiality of all residents.
Follows and upholds all house guidelines.
Cook meals as necessary for residents based upon food specifications, diets, food exchanges
and documentation.
Understands process for food supply and storage.
Transports residents and attends appointments with residents and advocates and obtains
necessary documentation, when needed.
Utilizes resident funds as applicable to help residents budget when needed.
Completes required documentation before the end of shift, including, but not limited to progress
notes, miscellaneous notes, daily support logs, event reports, incident reports, LOA forms,
resident inventories, etc.
Assists with supervision of residents doing their laundry and completes facility laundry, as needed
per assignment sheet.

Coaches and assists residents on bedroom cleanliness per house guidelines.
Responsible for cleanliness of home site according to the assignment chart. (Inspection ready
at all times.)
Must remain awake and immediately accessible in all homes requiring 24/7 care.
Mental Health classroom training.
Administers medications as needed.
Hand off communication to next shift staff.
Performs other duties as assigned or requested.

- High school diploma or GED strongly preferred, but not required
- 1-2 years of previous experience preferred, but not required.
- Excellent communication skills, both verbally and in writing.
- Must possess a valid Driver’s License.
- Ability to work in an environment with the potential for exposure to physical aggression
from individuals served. - Ability to physically and emotionally work with individuals who possess mental illness and co-occurring disorders.

Pay Range:USD $16.60 - USD $16.60 /Hr.Shift and Schedule:6 - 12 Hour Day Shift, 7 - 12 Hour Night ShiftSchedule:3 - 12.5 shifts with rotating weekends.
