Lead and direct case management services, workflow, and resources.
- Formulate and implement operational strategies and initiatives in the case management program.
- Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives.
- Provide specialized expertise, ensure resource alignment with system-wide initiatives, and update appropriate parties on case management strategies, initiatives, outcomes, and issues.
- Successfully partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care.
Licensure / Certification / Registration:
- One or more of the following required:
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date.
- Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
- Case Manager credentialed from the American Case Management Association (ACMA) preferred.
- Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
Education:
- High school diploma/GED with 2 years of experience, Associate's degree, or Bachelor's degree required.
- Master's degree preferred.
Work Experience:
- 5 years of experience required.
- 10 years of experience preferred.
- 2 years of leadership or management experience required.
- 5 years of leadership or management experience preferred.
Case Management experience preferred.
