About the Job
The Director of Case Management is responsible for directing, coordinating, and managing the departmental budget. Plans educational activities as well as ensures the effective delivery of Case Management services. Coordinates the integration of the department’s patient care and discharge planning process with related departments and external agencies. Moreover, ensures compliance with Hospital policies and patient care quality standards associated with regulatory bodies.
Responsibilities:- Directs, coordinates, and supervises the operation of the Department of Case Management. Monitor the department’s service activities and resource utilization and takes appropriate action as necessary.
- Directs and participates in the preparation of the annual budget for the Department in accordance with hospital’s budget guidelines. Reviews the Department’s budget performance on a monthly basis and develops an action plan for variances. Takes appropriate action to resolves budget problems and plans effectively for future budgetary needs.
- Initiates the development and implementation of departmental goals, policies, and procedures in accordance with the established standards of case management practice, Hospital goals and policies.
- Directs and participates in the selection of an information system designed to evaluate productivity and service effectiveness. Guides the preparation of departmental data collection and analysis activities and dissemination of reports both internal and external to department.
- Directs the Human Resource Management function in the department. Coordinates the selection, promotion, orientation, and performance appraisals of staff; and ensures that all related actions are managed equitably and in accordance with established Hospital and departmental policies. Recommends termination for both probationary and non-probationary employees. Facilitates employee counseling related to performance issues, complaints and/or conflicts, and refers employees to the Department of Human Resources if necessary.
- Directs and participates in the design and implementation of the organizational structure for the Department to meet current or future requirements. Defines new position responsibilities and reporting relationships.
- Provides supervision to Case Management staff in managing their respective service(s) and provides direction in managing both day-to-day and special clinical and administrative situations.
- Directs, coordinates, and controls the delivery of patient care within the department’s preview, and ensures compliance with established department and Hospital standards including standards as defined by the various regulatory agencies. Directs the quality improvement activities for the department and promotes the delivery of quality patient care.
- Directs and coordinates the Department’s discharge planning process in conjunction with related departments to facilitate continuity of care.
- Coordinates the provision of information and referrals to patients and their families regarding the psychosocial aspects of patient’s adaptation to illness and post-discharge needs.
- Directs and participates in the establishment and maintenance of effective working relationships with appropriate community service organizations and public agencies.
- Directs and coordinates the integration of the department’s functions with other aspects of overall patient care plans. Assists Hospital staff in addressing social service issues and related problems. Participates in the coordination of interdepartmental and Hospital program planning efforts.
- Directs and participates in the Department’s educational programs for Hospital staff and students to enhance both their understanding of the psychosocial aspects of patient care and professional development. Coordinates linkages with the schools of social work to ensure ongoing collaboration as regards student training. Directs and participates in research activities related to Case Management.
- Represents the Department of various hospital committees and in various professional meetings.
- Perform other duties as assigned.
Education:
- Master's Degree in Nursing (MSN) required.
Experience:
Minimum three (3) years of progressive Case Management leadership experience.
Knowledge and Skills:
- Strong technical knowledge of the area(s) being managed.
- Ability to establish goals, set priorities, oversee the short and long term deliverable of department objectives, ensure optimum performance of staff, may operate in a project manager role as needed, coach others, ensure quality of results and output.
- Solid understanding of the operation of the assigned department.
- Ability to fully utilize BHMC’s electronic record systems, equipment and other healthcare and billing systems relevant to this position.
Licenses, Certifications:
- New State Registered Nurse License required.
Physical Requirements:
- Position requires prolonged periods of standing, reaching, walking throughout the working day.
- Position will be required to stoop, bend, lift and carry items weighing up to 25 pounds.
