About the Job
The Director of Case Management is responsible for organizational care management and supervising the day-to-day operation of the Case departments at Baptist Health Care. Manages activities necessary to ensure appropriate utilization of the Hospital and its resources while maintaining optimal achievable standards of patient care. Maintains the strictest confidentiality of all patient information. Promotes the concept of cooperation and develops open lines of communication with department team members, patients, visitors and other personnel within and outside the Hospital. The Director of Case Management provides a key role in facilitating long-term success of care management by providing oversight of Baptist, Gulf Breeze, and Behavior Medicine at the corporate level. The Director Case Management will work in collaboration with the Physician Advisors to provide support to the case management process regarding patient discharge planning and transition of care. Maintains awareness of clinical documentation integrity activities ensuring that all aspects of the patient care continuum are addressed. The Director Case Management is responsible for increasing program awareness to the entire hospital staff, empowering Case Manager and Social Worker roles, integrating with Physician Advisors, establishing effect unit rounds. The person in this position works under general supervision, is responsible for various shifts, may be subject to over 40 hours per week and/or callback as required, and may also be required to remain on campus immediately before, during, and after severe weather and/or disasters.
Qualifications:- MBA or MHA or BSN is required.
- Requires five (5) years of hospital/healthcare experience in a case management including one year of leadership experience is required.
- If education is a BSN, a current RN licensure in the state of Florida is required.
- Ability to plan, coordinate, direct, and evaluate a variety of activities and projects
- Working knowledge of managed care and payer issues and knowledge of case management principles.
- Demonstrated critical thinking skills.
- Excellent interpersonal communication and negotiation skills.
- Strong analytical, data management and PC skills.
