Director of Care Coordination - MRINetwork Jobs

Reid & Associates has been engaged to conduct a highly confidential search for a Director of Care Coordination on behalf of a highly respected Medical Center in Sacramento, California. This position oversees 50 FTEs and will provide leadership in areas such as clinical resource management, discharge planning, patient advocacy, and best practices for medical necessity determination and concurrent review.

Position Overview:
The Director will guide Care Coordination and Social Services within the facility, ensuring alignment with the medical center's standards, as well as federal, state, and Dignity Health policies. The role involves collaborating with internal and external stakeholders to improve patient transitions through the healthcare continuum, overseeing performance improvement, and streamlining processes to enhance patient care and outcomes.

This highly visible role requires a leader with excellent communication skills, adept at addressing complex issues and educating staff. The ideal candidate will be responsible for ensuring regulatory compliance, guiding interdisciplinary collaboration, and driving key operational initiatives within the organization.

Compensation and Benefits:
The annual salary range for this position is approximately $139,717 to $213,123, based on experience and qualifications. Additional compensation options include a sign-on bonus and relocation assistance. Qualifications:

• Bachelor’s degree in nursing. (may consider MSW)
• Minimum of three years of progressive management experience in an acute hospital setting.
• Active CA RN license
• CM certification preferred.
• MSN Preferred
