Reid & Associates has been engaged to conduct a confidential search for a Director of Care Management for a dynamic healthcare system.

Role Overview: This role oversees the day-to-day operations of inpatient Case/Care Management across multiple facilities across Northern California, ensuring efficient patient flow from admission to discharge. The Director is responsible for system-wide Case Management, Care Coordination, and Discharge Planning, aligning these functions with strategic objectives.

Key Responsibilities:

• Oversee and optimize Care Management processes and metrics, including LOS, readmissions, and utilization.
• Ensure compliance with CMS, CA DCHS, Joint Commission, and other regulatory bodies.
• Manage inpatient Care Management policies and collaborate with Physician Advisors on audits and reviews.
• Provide leadership in recruiting, training, and coaching Care Management staff.
• Coordinate with Revenue Cycle, Utilization Review Committee, and quality leadership to enhance patient care and satisfaction.

Qualifications:

• Master’s degree in Nursing, Social Work, or related field.
• 7–10 years of case management or quality experience, including progressive management in a large healthcare organization.
• Active CA RN or Clinical Social Worker license, BLS certification required.
