About the Company: Our client is a fast-growing healthcare organization providing value-based care to frail and medically complex patients. Their mission is to revolutionize healthcare by improving patient outcomes through strategic partnerships and innovative care.
About the Job: The Director of Case Management will develop and implement case management strategies, oversee a team of care coordinators, and ensure staff are trained in value-based care (VBC). Hands-on leadership and skilled nursing facility (SNF) experience are essential.
What You’ll Do:
- Lead case management strategy and team development.
- Train and manage 15-20 case managers and care coordinators.
- Oversee transitions of care and patient-family engagement.
- Collaborate with clinical leadership for VBC improvements.
What You’ll Bring:
- Licensed RN (LVN considered for stellar candidates with VBC and SNF experience).
- Strong managerial experience with direct reports.
- Expertise in VBC and case management.
- Excellent communication and problem-solving skills.
Location: Hybrid role, with at least two days per week in the Costa Mesa office.
Compensation & Benefits:
- Base salary: $120,000 - $150,000 (potential for higher with experience).
- Annual incentive plan (AIP) up to 10%.
