Your Impact as a Director of Case Management:

As a Director of Case Management, you will:

• Oversee the entire Case Management Department, ensuring efficient and effective operations to achieve optimal patient outcomes.
• Lead and mentor a team of case managers, fostering a collaborative and supportive work environment.
• Develop and implement strategies to improve utilization management, transition management, and care coordination processes.
• Ensure compliance with state and federal regulations, Joint Commission accreditation standards, and hospital policies.
• Drive positive change through data analysis, performance improvement initiatives, and physician education.
• Collaborate with key stakeholders across the organization to promote efficient patient throughput and positive financial outcomes.

Qualifications:

• Required:
  • Bachelor's degree in Business, Nursing, or Healthcare Administration (for RNs) or Master's in Social Work (for MSWs)
  • 3+ years of acute hospital case management or healthcare leadership experience
  • Active Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW/LMSW) license for the state of California
• Preferred:
  • MSN, MBA, MSW, or MHA
  • 5+ years of acute hospital case management leadership experience, preferably in a multi-site setting
  • Accredited Case Manager (ACM) certification
