Job Details
We are seeking a Director of Case Management who will be responsible for leading a team of case managers and social workers in providing high-quality care to our patients. The ideal candidate will have experience in leadership & case management within an Acute or Post-Acute setting.
As the Director of Case Management, you will be responsible for ensuring that our patients receive the best possible care, while also managing the day-to-day operations of the department.
Responsibilities:
- Provide leadership and direction to the case management team, ensuring that all team members are working together to provide high-quality care to our patients.
- Oversee the development and implementation of care plans for all patients, ensuring that they are tailored to meet the individual needs of each patient.
- Work with other members of the healthcare team to ensure that patients receive the appropriate level of care, and that all care is delivered in a timely and efficient manner.
- Develop and implement policies and procedures to ensure that the case management department is operating in compliance with all relevant regulations and guidelines.
- Manage the budget for the case management department, ensuring that all resources are being used effectively and efficiently.
- Provide ongoing training and development to the case management team, ensuring that they have the skills and knowledge necessary to provide the best possible care to our patients.
- Participate in quality improvement initiatives, ensuring that the case management department is constantly striving to improve the quality of care that we provide to our patients.
- Serve as a liaison between the case management department and other departments within the organization, ensuring that all departments are working together to provide the best possible care to our patients.
Qualifications:
- Bachelor's degree in Nursing (BSN) or Social Work (LCSW) required;
- Experience in case management required
- Previous leadership experience required
- Experience in acute, post-acute, or rehabilitation strongly preferred.
- Strong leadership skills, with the ability to motivate and inspire a team to provide the best possible care to our patients.
- Excellent communication skills, with the ability to communicate effectively with patients, families, and other members of the healthcare team.
- Strong analytical and problem-solving skills, with the ability to identify and address issues before they become major problems.
- Knowledge of relevant regulations and guidelines, including Medicare and Medicaid regulations.
- Ability to work effectively in a fast-paced environment, with the ability to manage multiple priorities and deadlines.
