Director of Case Management
We are seeking a dedicated and experienced Director of Case Management to join our team at Aston Health, a skilled nursing and long-term care (SNF/LTC) facility. This role will be responsible for overseeing the case management team, ensuring the delivery of high-quality patient care, and coordinating comprehensive discharge planning. The ideal candidate will have a strong background in case management within SNF and LTC settings and a commitment to improving patient outcomes.
Director of Case Management Responsibilities:
- Lead and manage the case management team, providing guidance, support, and professional development opportunities.
- Oversee patient assessments, care planning, and discharge processes to ensure continuity of care and adherence to regulatory standards.
- Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and rehabilitation staff, to develop and implement individualized care plans.
- Monitor and evaluate patient progress, making adjustments to care plans as necessary to optimize clinical outcomes.
- Ensure compliance with federal, state, and local regulations related to case management, discharge planning, and patient rights.
- Develop and implement policies and procedures for case management activities, ensuring best practices and quality standards are met.
- Coordinate with insurance companies, third-party payers, and other external organizations for authorization and reimbursement processes.
- Track and report key performance indicators related to case management, including readmission rates, length of stay, and patient satisfaction.
- Facilitate family meetings and provide education on the patient's condition, care options, and available resources.
- Serve as a resource for complex case management issues, offering expertise in navigating the healthcare system and resolving barriers to care.
Director of Case Management Qualifications:
- Bachelor’s degree in Nursing (BSN), Social Work, or a related field; Master’s degree preferred.
- Active RN license, LMSW, or LCSW, depending on state requirements.
- Minimum of 5 years of case management experience in a Skilled Nursing Facility or Long-Term Care setting.
- Previous leadership experience in case management or a related field is strongly preferred.
- Knowledge of Medicare, Medicaid, and other payer systems, including coverage guidelines and reimbursement processes.
- Strong understanding of regulatory requirements, including CMS guidelines and state health regulations.
- Excellent communication, leadership, and organizational skills.
- Certification in Case Management (CCM or ACM) is a plus.
