Director of Healthcare Services at CrownPointe of Indianapolis

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team and serve as the Director of Healthcare Service aka DON.  As HCSD or DON, you will play a crucial role in providing high-quality care to our residents in our newly remodeled beautiful assisted living. This is an excellent opportunity for a experienced supervisor to utilize their expertise and make a positive impact on the lives of residents.

Responsibilities include but are not limited to the following:
- Lead, guide and educate the clinical team in all resident care aspects.
- Admission and other assessments
- Administer medications and treatments as prescribed
- Monitor patient vital signs and report any changes or abnormalities
- Monitor and manage departmental scheduling and budget
- Maintain accurate and up-to-date medical records
- Collaborate with other healthcare professionals to develop and implement residents service plans
- Educate residents and their families on healthcare procedures, medications, and self-care techniques.

Qualifications:

- Valid LPN licensed in the state of IN
- Minimum of 1 year experience long term care or assisted living
- Minimum of 1 year of clinical supervisory experience
- Proficient in managing medical records and documentation
- Ability to assess patient needs and develop appropriate care plans
- Excellent communication and interpersonal skills
- Demonstrated leadership abilities in coordinating patient care
- CPR/First Aid Certified
- Ensure compliance with all applicable laws, regulations, and policies
