Director of Intake Opportunity Description

Our client is a national provider of Home Health and Hospice services. They have a current opening for a Director of Intake based in the Jericho, NY area.  An RN with Medicare-certified Home Health leadership experience is required. Must understand intake processes, care coordination, and referral source management. 

Director of Intake Job Requirements

• Medicare certified home health leadership experience.
• Intake / Care Utilization / Referral Source Management experience.
• Clinician required (RN)
• Track record of business development

Director of Intake Job Responsibilities

• Day-to-day oversite of centralized intake department.
• Coordinate proper placement of patients and appropriate level of care.
• Communicate with discharge planners, physicians, referral sources, and patients.
• Develop department / improve procedures and processes.
