Job Description:

Total Care Home Health, a brand-new division of Total Care Connections, is looking for a Director of Nursing who is ready to roll up their sleeves and make an impact from day one.
As the Director of Nursing, youll take a pivotal role in launching our Medicare-certified home health division, overseeing both clinical and operational needs. In addition to managing compliance and quality assurance, this role requires direct patient involvement, especially in the early stages as we build our census. This includes conducting patient visits, opening cases, and ensuring every new client receives extraordinary care. Key Responsibilities:
- Startup Leadership: Lead the launch and development of a brand-new Medicare-certified home health division, including policy creation, initial hiring, and operational processes.
- Direct Patient Care: Conduct patient visits, open cases, and manage clinical care when needed as the census grows.
- Clinical Leadership: Establish and oversee all clinical operations, setting policies and standards that ensure high-quality, compliant care for both adult and pediatric clients.
- Regulatory Compliance: Maintain agency readiness for Medicare, Arizona Department of Health, and ACHC accreditation, overseeing all regulatory surveys and audits.
- Quality Assurance: Drive quality improvement initiatives focused on patient safety, satisfaction, and positive clinical outcomes.
- Staff Development: Recruit, train, and retain clinical staff, creating a strong, collaborative team with clear expectations.
- Pediatric & Adult Program Oversight: Develop and implement care protocols that meet the unique needs of both pediatric and adult clients.
- Continuous Improvement: Spearhead continuous improvement efforts to enhance efficiency, patient experience, and clinical performance.
Qualifications:
- Experience: Minimum of 3 years in clinical leadership within a Medicare-certified home health setting, with experience managing both adult and pediatric home health services.
- Startup Readiness: Willingness to perform patient visits and case openings as part of the growth strategy.
- Clinical Knowledge: Deep understanding of Medicare and Arizona Department of Health regulations, specifically within home health care.
- Leadership Skills: Strong leadership abilities to motivate and develop a high-performing clinical team.
- Communication & Organization: Excellent organizational skills with a focus on clear, proactive communication.
- System Proficiency: Familiarity with EHR systems, ACHC accreditation processes, and ideally, the EOS (Entrepreneurial Operating System).
Education and Credentials:
- Bachelors degree in Nursing (BSN) required; Masters in Nursing or Healthcare Administration preferred.
- Current, unrestricted Arizona RN license.
- Certification in home health or healthcare administration is a plus.
