Job Description
Purpose:
Under the general direction of the Chief Growth Officer and the Chief Nurse Officer, is responsible for the direction, coordination and control of clinical and functional activities of the Ambulatory Care Service. The Director of Nursing in the Ambulatory Care Service is responsible for providing clinical oversight and directions to the nursing team.
Specific Duties and Responsibilities:
• Assumes responsibility for the daily operations of the Ambulatory Care nursing services.
• Collaborates with the Chief Growth Officer, Chief Nurse Officer and Ambulatory Care Administration in planning, directing, monitoring and evaluating aspects of preventive measures and chronic illness care delivery and maintains communication with Interdisciplinary care teams.
• Keeps abreast of current nursing technology and professional techniques and disseminates latest developments in the field to nursing staff. Ensure staff development, education and growth, realigning staffing models, optimizing throughput/cycle time, enhancing care models and pre-visit planning.
• Integrates nursing functions with other clinical and operational programs, thus assuring efficient and effective operations.
• Interprets policies and objectives of nursing services to staff and promotes harmonious working relationships with other departments.
• Directs, coordinates and controls the execution, implementation and revision of departmental policies/procedures to assure attainment of department goals/objectives.
• Assumes responsibility for selection and evaluation of nursing personnel.
• Facilitates and chairs team meetings, encouraging active participation and clear communication of team goals, objectives and performance.
• Actively participates in facility and central office lead initiatives, communicating feedback to members of the interdisciplinary team.
• Works closely with the clinical improvement staff of the Office of Population Health, and SBH stakeholders to lead, communicate, and shares experience with other facility coordinators. Leads improvement efforts to improve quality of care and optimize chronic disease care delivery, prioritizing equity and social determinants of health. Works to actively pursue higher accreditations, certifications and centers of excellence.
• Works with the interdisciplinary team in the identification of care gaps and resolutions to address areas for improvement.
• Encourages utilization of PCMH model thus, involving and keeping the patient at the forefront of his/her care.
• Monitors patient experience with the goal of identifying strategies that focus on providing exceptional patient care.
• Facilitates participation in monthly conference calls with the Office of Health Care Quality and Clinical services.
• Attends and or participates in quarterly meetings with the Ambulatory Care Directors and Office of Health Care Quality at Central office.
• Identifies training and skills development needs of staff, and facilitates that training in order to strengthen care rendered to patients in the ambulatory care setting.
• Implements a training schedule for the Ambulatory Care staff to establish ongoing education regarding the Chronic Care Model and the corporate best practices established by Collaborative team testing.
• Participates in oversight of ACO activities; proactively identifies and resolves obstacles to implementing spread activities in the clinical areas.
• Collaborates with provider staff, nursing staff and administrative staff to eliminate barriers to the implementation of group and planned visits.
• Oversees the Population Health Dashboard: provides oversight for report preparation or prepares reports on Population Health Dashboard
• In collaboration with interdisciplinary team leads, monitors clinical performance of the team and provides constructive feedback in order to enhance team performance.
• Monitors and provides follow-up on patient care incidents to ensure adequate reporting and corrective actions as necessary.
• Identifies areas for improvement in the ambulatory care service and initiates and monitors performance improvement activities by engaging staff at all levels.
• Analyzes the results of CAHPS surveys and other forms of patient feedback to identify strengths and weakness.
• Participates in accreditation, Pl and TJC and regulatory activities. Ensures Regulatory Agency Survey Readiness. Implements patient safety goals, regulatory survey preparation and specialized training initiatives and continued education.
• Collaborates with All Ambulatory Leadership on Projects as needed and assigned.
• Develop and support structure for Transitional Care Management in Primary Care.
• Lead Nursing Epic Optimization, attend meetings, track nursing specific issues and concerns, and develop best practices.
• Identifies and implements cost containment and revenue enhancement activities. Responsible for optimal inventory management and new cost containment strategies for timely access and management of required supplies and savings.
• Oversees Professional Shared Governance activities as facility lead.
• Additional duties as assigned by Chief Nursing and Growth Officers.
Minimum Qualifications
1. Licensed and currently registered to practice as a Professional Nurse in New York State; and,
2. A Masters Degree in Nursing Administration or an equivalent field, and a minimum of four years of high-level responsible experience in nursing administration, three years of which shall have been in a hospital; or,
3. A satisfactory equivalent. However, all candidates must be licensed and currently registered to practice as a Professional Nurse in New York State.
