Schedule Monday - Friday 9am- 5pm

SUMMARY: The Director of Nursing is responsible for the coordination, case management, and oversight of health services for individuals within the Agency’s 24-hour, Residential Services Division as regulated by the Department of Developmental Services (DDS), while ensuring and safeguarding individuals’ human rights, dignity, and liberties. The Director of Nursing provides supervision and leadership to Nursing Department Case Managers and the MAP Training Coordinator and also provides oversight of program staff and individual healthcare trainings, including Medication Administration Program (MAP) training.

ESSENTIAL FUNCTIONS

Leadership

• Act as primary leader of nursing/health services within the Residential Services Division and collaborate with other members of management/leadership within other Agency programs
• Educate employees to develop focus on individuals’ health and wellbeing
• Provide direction and supervision to direct-report Nursing Department staff
• Oversee nursing process-related component of program nursing positions
• Conduct weekly department meetings with direct reports, as well as semiannual meetings with other program nursing staff
• Act as chairperson of Agency’s MAP Committee

Healthcare Services

In collaboration with Nursing Department team members, the Director of Nursing will:

• Monitor the general health and well-being of program individuals and engage in appropriate follow-up to ensure individual health needs are being met
• Conduct routine and as-needed nursing assessments on individuals
• Conduct preventative healthcare assessments annually to ensure individuals receive all recommended preventative care in alignment with DDS Adult Screening Recommendations
• Assist with health-related communication between healthcare providers, families, DDS, and other stakeholders, as necessary
• Assist program management in Individual Support Plans (ISP) meetings to assist with developing medical status assessments, as required
• Collaborate with program management to ensure healthcare orders, Medication Administration Records, healthcare protocols/guidelines, Medication Treatment Plans, Supportive and Protective documents, and all other documentation remain in compliance with applicable laws, policies, and regulations at all times
• Collaborate with program management to ensure individual Healthcare Records (HCRs) are accurate and remain up-to-date in the Home and Community Services Information System (HCSIS)
• Collaborate with program management to prevent falls via fall prevention plans and training
• Collaborate with program management in the admission and discharge process to ensure individuals receive appropriate healthcare follow-up and advocacy.
• Identify potential need in creation of healthcare proxy, indication for change in medical code status, or beneficial services, such as palliative or hospice care
• Oversee biannual audit process to evaluate healthcare services and MAP compliance within all residential programs; create a plan for immediate corrective action needs; and identify trends requiring systemic change.
• Screen program referrals for medical support needs
• Manage medication stock, personal protective equipment, and vital signs supplies, ensuring adequate reserve is available at all residential locations
• Provide oversight and promote nutrition/healthy-eating and create/maintain standardized medical diet plan
• Draft new and update existing residential healthcare policies, protocols, procedures, and forms in compliance with Department of Public Health (DPH) and DDS regulations. Ensure nursing staff function in accordance with corresponding Scope of Practice and the Commonwealth’s Nurse Practice Act
• Attend Residential Leadership, Human Rights Committee, and DDS Risk meetings, as well as other meetings as requested
• Be responsible for the maintenance and renewal of Agency Automated External Defibrillators (AEDs), including but not limited to Residential homes and all Agency administrative offices. Manage equipment and supplies and ensure expiration dates are managed closely to certify that AEDs remain in compliance and operational at all times.

MAP & Healthcare Training

• Provide supervision of MAP Training Coordinator, who is responsible for overall MAP training of Residential staff, to ensure an effective and efficient MAP training program
• Oversee healthcare training program and curriculum of Agency new employee orientation, including, but not limited to, universal precautions, blood-borne pathogens, and Hepatitis exposure, as well as identify and assist with other training projects, as needed
• Oversee healthcare training process to ensure current Residential staff are trained and knowledgeable about specific medical needs of individuals they support. Some examples may include training staff on individual-specific healthcare protocols/needs (ex. diabetes Director of Nursing – Position Description Page 3 of 4 management protocol, low cholesterol diet, etc.), skills (ex. glucometers, g-tube medication administration, etc.), and specialized medications (ex. warfarin, glucagon spray, epi-pen, etc.)
• Create MAP reference guide for certified staff
• Create new nursing orientation program for new-hire nurses, which includes pertinent information necessary for nurses working within MAP programs

Administrative

• Oversee scheduling of after-hours, on-call nursing; serve within on-call rotation and backfill direct report responsibilities as needed
• Ensure direct-report nursing staff schedules remain updated in scheduling database at all times.
• Review monthly Medication Occurrence Report (MOR) data to identify trends and need for follow-up to reduce future MORs
• Act as liaison between the Residential Services Division and community healthcare services (ex. pharmacies, VNAs, physical therapy, etc.) to address the needs of programs and its individuals.
• Develop relationships with administrative teams within local hospital/healthcare systems to ensure proper coordination and care of program individuals receiving services
• Review and approve C.N.A. certification renewal
• May be required to use employee’s personal vehicle for work purposes.
• Perform other duties as assigned by the Vice President of Residential Services or COO, including, but not limited to, working weekends and/or holidays
• Participate in all staff meetings and required training sessions, including in-service training, as scheduled
• Participate in all required program and Agency trainings

QUALIFICATIONS:

• Current unrestricted licensure in Massachusetts as a Registered Nurse (RN), with a minimum of a Bachelor’s of Science Nursing (BSN) degree or equivalent, masters preferred.
• 3+ year experience as Registered Nurse
• 3+ years’ experience in supervisory role
• 2-5 years’ experience with DPH Medication Administration Program (MAP) and population served, preferred
• MAP Trainer certification through Department of Developmental Services/DPH, preferred.
• Skill in consulting with medical professionals and advising others on medical and/or health-related issues.
• Skill in building, motivating, and leading teams.
• Ability to maintain reliable transportation and travel to local areas to include but not limited to: Fall River, Somerset, Swansea, Westport, Taunton, and Mansfield.
• Ability to conduct Medication Administration Program training sessions, as needed.
• Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists.
• Ability to read, analyze and interpret medical documents, professional journals, technical procedures and/or operational regulations.
• Ability to prepare reports, business correspondence and procedure manuals.
• Ability to maintain confidentiality and HIPAA standards.
• Ability to work independently and as a team.
• Ability to work and communicate effectively.
• A qualifying criminal background check, driving record and a valid driver’s license required.
• Ability to provide current copy of all required health records and TB assessments.
