Job Description:
Our client is seeking a Director of Social Services. You will oversee the Social Services Department, focusing on the needs of short-term rehab residents and discharge planning. As the Director of Social Services, you will work closely with the Assistant Director of Social Services to ensure the effective delivery of social services and support to residents
Qualifications:
· The job requires a bachelor’s degree in social work, at least 1 year of supervised experience in healthcare, and Massachusetts Licensure
· Accepted Licenses: LSW, LCSW, LICSW
· Not Accepted License: LSWA
Job Responsibilities:
· Assist in planning, developing, organizing, implementing, evaluating, and directing the facility's social service programs.
· Participate in discharge planning, resident assessments, and developing social care plans.
· Interview residents/families to obtain social history.
· Refer residents/families to appropriate community agencies when needed.
· Provide consultation to staff and community agencies on meeting residents' social/emotional needs.
· Interpret residents' social, psychological, and emotional needs to the care team.
· Assist in obtaining resources from community agencies to meet residents' needs.
· Ensure confidentiality of residents' protected health information.
Administrative Duties:
· Assist in developing and administering department policies and procedures.
· Review policies/procedures annually and recommend changes.
· Complete required documentation, forms, and reports.
· Assist in arranging transportation for residents when necessary.
· Provide information on financial assistance programs to residents/families.
· Assist in reviewing and updating job descriptions annually.
· Delegate authority and make recommendations to the Director.
· Standardize methods and provide solutions for residents' social/practical needs.
