JOB SUMMARY
Under general supervision of the Administrator, the Director of Social Services is responsible for reviewing treatment/care plans and services for members of the Short Stay Unit and Long Term Care custodial setting to ensure the efficient use of patient utilization systems and quality care using appropriate medical resources. Also serves as a liaison between the skilled facility, physician, social worker and other applicable team members, to authorize and arrange any ancillary and specialty services the member may require to effectively coordinate their care including documenting and communicating any transitions to alternative levels of care. The Director will also evaluate the quality of care the facility is providing and its conformance with organizational policies, procedures and guidelines, as well as compliance with government regulations. The Director will supervise the entire Social Services department assuring appropriate utilization of facility resources and compliance with regulatory guidelines and practices. Works with staff, residents, vendors, contractors and the general public. Must be able to fluently communicate in or to take or give direction or guidance in the English language.
PRIMARY ACCOUNTABILITY
A thorough knowledge of the practices and procedures in the use and function of Case Management/Social Services. Strong written and oral communication skills. Effective in establishing and maintaining rapport with staff, residents, and resident representatives. Must possess the ability to maintain composure under pressure. A professional appearance is to be maintained at all times. The Director of Social Services will possess a genuine interest in and concern for geriatric, chronically ill, and/or disabled persons. Desire and ability to learn additional software used by Glencroft.
ESSENTIAL FUNCTIONS
- Regular attendance is considered an essential function of this position and is necessary for the efficient operation of the business.
- Oversees the entire social service and discharge planning process, to include participation in the interdisciplinary evaluation of residents needs for institutional care and completion of the PASARR.
- Reviews and analyzes medical information either through telephonically or onsite, to determine the medical necessity of continued stay according to custodial level of care review standards.
- Oversees the process of Advance Care Planning for each resident upon admission and makes sure that any Advanced Directives are reviewed with the resident/resident representative on a regular basis.
- Adjudicates and communicates custodial care authorizations to providers.
- Participates in the development of a patient care plan in a long-term care environment that incorporates patient centric choices.
- Initiates/coordinates and oversees the discharge planning processes of the facility with physicians, members, families, caregivers and ancillary providers to support the member’s continuity of care needs when they are going to an alternate level of care.
- Collaborates with all Departments and community-based organizations in planning de-institutional transition activities for members returning to the community.
- Coordinate implementation and oversight of procedures to ensure social services actions and interactions are adequately documented in each resident’s medical record.
- Assist residents in voicing and obtaining resolution to grievances.
- Promotes effective communication, education, problem resolution and consistency of application of utilization/authorization criteria and function pertaining to facility patients and residents.
- Assist residents and their representatives in locating and accessing financial, legal and other community resources.
- Utilizes monthly reports to track and monitor custodial cases for ongoing care coordination and authorization requirements.
- Attends/Oversees and facilitates case conferences on an as needed basis with internal and external professional staff, social services representatives, family members, external case managers and other community resources.
- Participates in Case Management meetings, utilization review, and managed care meetings. Reviews and coordinates patient care daily to assure quality care, qualification for insurance/Medicare coverage, proactive discharge planning, care coordination with provider representatives.
- Responsible for all insurance interactions on a timely basis, that is by phone, email or fax.
- Assist admission in reviewing referrals and coverage when the admission director is out of the facility.
- Excellent verbal and written communication skills.
- Exemplary patient advocate skills.
- Knowledgeable in the continuum of care across all settings.
- Other duties as assigned.
