As the Director Social Services you will assist with marketing, admissions, discharge, and transfer needs. Under the direction of the Administrator, you will assist in the planning, organization, and development of residents to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis. You will also be responsible for maintaining written documentation in the resident medical records per facility policy and state and federal guidelines.
Qualifications
- Must be a Licensed Social Worker in good standing.
- Completion of secondary education at an approved school of Social Work.
- Previous experience in supervisory capacity preferred.
- Knowledge of a specialized field such as geriatric, adult social work, or long term
- Must have the ability to relate professionally and work cooperatively with residents, families, employees, and other stakeholders at all levels of the organization and community to ensure continuity of care.
- Bachelor’s degree in social work. One year supervised social work experience working directly with individuals in a long-term or sub-acute care rehabilitation.
- Current Social Work license in the state of employment
- Working knowledge of personal computer and software applications used in job functions, (word processing, graphics, databases, spreadsheets, etc.)
- Strong organizational and analytical skills; oral and written communication skills.
- Working knowledge and ability to apply professional standards of social work practice in job situations.
- Current knowledge of state and federal laws and regulations that apply to the practice of social work in long-term care.
- Be strong advocate for the residents and families.

- Assessments & Care Planning
- Discharge Planning
- Use of PCC
- Care Planning
- MDS and Completion of CAA
- Scheduling Care Conferences at 72 hrs. post admission, quarterly and PRN
- Grievance Officer
Abuse Coordinator
