Oversee and manage the social services department, ensuring compliance with all regulations and standards.
Conduct assessments and develop individualized care plans in collaboration with interdisciplinary teams.
Advocate for residents’ rights and assist them and their families in navigating the healthcare system.
Provide counseling and support to residents and their families, addressing emotional, social, and financial challenges.
Organize and facilitate group activities and programs to promote social engagement and community involvement.
Collaborate with external agencies and community resources to support residents’ needs.
Maintain accurate and confidential documentation in accordance with regulatory requirements.
- 1 Year SNF/LTC Healthcare experience.
- Social worker license in PA.
