Job Description:
The Director of Social Services and Admissions fulfills a combined role of Social Services and Admissions/Marketing. This position uniquely follows residents by supporting them through their individual continuum of care beginning at admission and throughout their transition.
Social Service Functions:
The Director of Social Services and Admissions is responsible for implementing and documenting a plan of care related to the resident’s social/emotional needs in the electronic medical record. The Director of Social Services and Admissions is also responsible for collaborating with the nursing, MDS and rehabilitation departments to facilitate discharge plans for residents. The Director of Social Services and Admissions is responsible for informing and educating the staff/resident/family/responsible party of the resident’s personal and property rights, as well as his/her rights related to health services as pertaining to the resident. The Director of Social Servicesand Admissions maintains updated resources for residents and families in regards to community resources and services, as well as service agencies and organizations. Maintains a record of all reported concerns, assists in resolving them and assures appropriate follow up and documentation. The Director of Social Services and Admissions is easily accessible to residents, family and staff. Provides timely referrals to appropriate agencies for assistance to the resident/family (i.e. conservatorship application, Medicaid application, etc).
Admissions Functions:
Organizes, controls, and executes the admission process including referral management, community/hospital sales, admitting new residents, and completing admission agreements in compliance with all regulations and guidelines. Coordinates facility specific and supports company activities including attending external community marketing events, and arranging tours of the facility in accordance with facility and corporate guidelines.
Coordinates all admissions to facility in accordance with budgeted census projections and maintains budgeted census mix. Conducts follow ups on all inquiries/tours, and manages bed availability with team in order to accept most admissions possible. Attends pertinent facility meetings, with correct, up to date referral and resident information. Verifies all payers for active coverage, available benefits, contracted plans and requirements prior authorization requirements. Completes the notification process and reporting requirements as directed by regulatory authorities and facility policies. Responsible for collecting the correct PASRR from the hospital or community physician during admission process. Communicates information to the resident and family concerning facility admission agreement, residents’ rights, family and resident responsibilities, conditions of admission, and licensing and regulatory requirements.
Marketing Functions:
Facilitates the development of marketing strategies, public relations activities and programs designed to promote the facility and the entire company. Drives marketing and events, as well as improves hospital relationships to increase referrals. Acts as liaison between facility and all community group agencies. Attends 5 external community marketing visits/events per quarter. Hosts 2 internal marketing events per year (events specifically for resident/family/employees not included)
Qualifications:
- Bachelors or Masters Degree from an accredited program in Social Work or related field.
- Previous experience in a long term care facility as well as with current Requirements of Participation regulations is preferred.
- Must be knowledgeable of admissions and discharge procedures in conjunction with all current laws, as well as be familiar with residents’ rights and documentation requirements in accordance with current regulations and/or guidelines.
- Attention to detail, good follow through skills and ability to prioritize multiple tasks.
- Must be innovative and able to find creative ways to serve the variety of resident needs and maintain the customer focus.
- Must have a vehicle and a valid drivers’ license.
- Demonstrates cooperation in assisting others and ability to work as a team.
- Patience, tact, enthusiasm and a positive attitude toward the residents and their support networks.
- Ability to remain calm in stressful situations.
- Ability to communicate with residents, staff and family members in a professional manner.
