Director of Social Work - Skilled Nursing Facility

Our Skilled Nursing Facility is hiring a Director of Social Work (DSW) or Director of Social Services in Brooklyn, NY.

Responsibilities:

• Develops, implements, and oversees policies and objectives for the Social Services Department
• Conducts resident assessments, care planning, and facilitates resident adjustment
• Supervises departmental staff and ensures effective operation
• Initiates discharge planning from the point of admission
• Engages in occasional community outreach activities
• Applies a range of concepts, practices, and methods to ensure quality care
• Exhibits a high degree of creativity and flexibility
• Leverages substantial experience and critical judgment to achieve departmental goals

Qualifications:

• Minimum 2 years of management experience in long-term care settings
• LSW or MSW required
• Solid knowledge of relevant state and federal regulations
• Strong interpersonal and leadership abilities
