Key Responsibilities:

• Responsible for coordinating all admissions and discharges to ensure facility census is kept at its maximum capacity.
• Participates in an interdisciplinary team approach to better serve the needs of the facility population.
• Implementing and documenting a plan of care related to the resident’s social/emotional needs in the electronic medical record.
• The Director of Social Services and Admissions is also responsible for collaborating with the nursing, MDS and rehabilitation departments to facilitate discharge plans for residents

Qualifications & Experience Requirements for Director ofSocial Work: 

• Previous Experience as a Director of Social Work preferred
• Must have SNF or LTC experience 
• Valid Social WorkerLicense 
• Must possess, as a minimum, a Master of Social Work from an approved school of Social Work.
• Must have a minimum of 3 years of long-term care experience.
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
• Strong leadership skills in a healthcare setting
• Strong written and verbal skills
