Special Requirements: This is for an Admission, Discharge, Transfer (ADT) position that focuses on patient progression and optimizing discharge workflows. The shift is approximately 0800-1830 and involves collaborating with the multidisciplinary care team to identifiy discharge related tasks earlier in the patients' stay and elminiating barriers to timely discharges.
