JOB SUMMARY
The Discharge Support Specialist is responsible for performing administrative and operational functions required in the Care Management Department to support regulatory requirements and patient throughput activities to ultimately ensure patients receive the treatment and services they require post-discharge and are informed of their rights and choice options during the process. The Discharge Support Specialist oversees the administrative operations of the Care Management Department and acts as the administrative liaison between all clinical departments, community resources, and third-party payers/insurance companies in support of the work of the department.

Familiar with Medical Terminology.

REQUIREMENTS
- Preferred High School Diploma Medical Terminology
- Preferred Associate's Degree
- 1-3 years Previous clerical experience in a healthcare setting. Experience with office equipment to include Microsoft Suite, and electronic medical record systems. Required
- Less than 1 year Experience in care coordination, Care Management, Disease Management, hospital/acute care setting, or customer service. Preferred
