This is a full time-4 day/36 hour workweek position
This position in is our Children & Family Services department will provide resource coordination to children enrolled in early intervention, family support services program and children's Medicaid waivers. Responsibilities include working with families to identify needs, develop individual family support plans, information and referral, and the coordination of services.
Qualifications:
B.A. required. Degree in Human Services or related field preferred. Must have experience with systems that serve people with developmental disabilities. Service coordination and experience with the developmental disabilities field strongly preferred. Ability to maintain flexible schedule and provide reliable transportation. Must demonstrate high degree of responsibility and good written and oral communication skills.
Physical Requirements:
Must have the Ability to lift/push/pull/carry 15 pounds, sit for up to two hours at a time throughout an 8-10 hour day, extensive use of the telephone and the computer and accessories, ability to speak clearly and distinctly and be understood by others, read written material, write/keyboard while listening and facilitating, and independently travel to meeting and monitoring sites. Ability to bend and stoop occasionally. Ability to reach to shoulder level and above. .
