POSITION SUMMARY: Identification, referral, and coordination of services for disabled children, ages birth to three, and their families, serving as a single point of contact for the families, and advocating for the children and families.
ROLE AND RESPONSIBILITIES
- Receive and process referrals within the program time frame
- Schedule and conduct routine-based interviews
- Develop an Individual Family Service Plan (IFSP)
- Maintain monthly contact with families
- Begin transition planning within the program time frame
- Connect families to local resources, serving as an advocate for them
- Maintain accurate records
- Participate in all required meetings and trainings
- Adhere to all CNCAP policies and procedures
QUALIFICATIONS
- Must have an AA or BA degree in Early Childhood, Family Studies, Social Work, or other
related field - Valid driver's license
- Previous experience working with children with disabilities and their families
SKILLS & ABILITIES

- knowledge of word, excel, powerpoint, and databases

- Demonstrated ability to work with others
SCHEDULE: Monday-Thursday: 7am - 5pm or Monday-Friday: 8am - 4pm
REPORTS TO: Area Manager/Education Coordinator
JOB STATUS: Full-Time, 40 hours per week
PAY: $18.75 - $20.41/Hour (Depending on qualifications)
