POSITION SUMMARY
An Engagement Specialist meets our clients at the crossroads of their mental and physical health to fill gaps and improve engagement and access to care. This position is an integral part of the First Episode Psychosis (FEP) program, Insight, and functions as a collaborative member of the client’s care team to facilitate linkage to appropriate services. The physical and mental health needs of the client are met through care coordination, support, education, and motivation. Porter-Starke Services is a community mental health center and certified community behavioral health clinic. Our full continuum of care serves adults, families, adolescents, and children with mental health or substance abuse issues. We treat all clients on individual plans of care, tailored to specific needs.

VIITAL DUTIES AND RESPONSIBILITIES
- Engages with potential Insight clients and/or families/supports to explain and introduce the Insight Program.
- Manages caseload of Insight clients receiving care management/coordination services
- Attends Inpatient Unit Staffing meetings to coordinate care for potential referrals to the Insight Program.
- Reaches out as a support to existing Insight clients when the client has been admitted to the Inpatient Unit or receives services through the Mobile Crisis Team or Crisis Center.
- Provides services to ensure/increase access to appropriate care
- Facilitates client access to primary/specialty healthcare, communication among providers, and referrals to appropriate resources and services within and outside of the agency.
- Works collaboratively with department coordinator, Data Management Team, Outpatient Treatment providers (therapists, doctors, and nurse practitioners), Case Managers, Health Mentors, community partners and other members of the client’s treatment team inside and outside of Porter-Starke Services.
- Collaborates to implement grant requirements, policies, and programmatic changes to develop sustainable improvements to care.
- Facilitates evidence-based groups and creates support opportunities for clients
- Utilizes assessment and treatment planning tools to maintain a client centered, goal-oriented approach to integrated health.
- Facilitates identification of high-risk or at-risk clients that may have unmet physical and/or behavioral health needs.
- Plans, organizes, and prioritizes multiple activities and maintains positive relationships with a wide variety of people; works collaboratively with all stake-holders.

QUALIFICATIONS:
- Bachelor Degree in Psychology, Social Work, Rehabilitation, Health Science, Public Health, or related field required.
- Experience and/or training in Motivational Interviewing and Motivational Strategy Building preferred.
CERTIFICATION/LICENSE
- Must possess a valid driver’s license.
- Must be CPR certified (training provided)
