JOB SUMMARY:
The Home Health Aide provides basic care to clients in the home environment including personal care, nutrition, exercise, ambulation, positioning, and transfers and must have excellent communication, organization, and time management skills. Working under the supervision of an RN, they must be able to follow a plan of care and deliver care according to current policies and procedures.
Candidates must demonstrate maturity, flexibility, and the ability to deal with potentially demanding situations and have a reliable source of transportation in order to fulfill the duties and responsibilities of this position.
RESPONSIBILITIES:
- Receives assignments from scheduling staff/team leader and is responsible for following through with all visits for each assignment at the pre-scheduled time and location.
- Follows each client’s individualized care plan, schedule, and assignment as directed, reviewing each plan of care prior to making the home visit.
- Communicates all pertinent information to the other staff involved in the client’s care, such as client and family requests, changes in health status, scheduled appointments, falls, injuries, suspected neglect or abuse, etc.
- Provides personal care, such as bathing in bed, tub, or shower as appropriate; dressing; transfers, including use of lifts; ambulation; range of motion exercise; oral hygiene; toilet needs; non-sterile dressing changes; and any other miscellaneous activities of daily living (ADL), which may include catheter and colostomy care.
- Ambulates clients needing assistance with walking, employing assistive devices such as gait belts, walkers, canes, and/or wheelchairs, as necessary, following proper lifting techniques to ensure the safety of clients at all times.
- Performs light housekeeping, such as vacuuming, mopping floors, dusting, changing bed linens, washing dishes, washing/drying/folding laundry, cleaning bathroom fixtures, etc.
- Shops for groceries or essential personal articles (only as pre-approved by Director) and organizes households.
- Records/documents all client data specific to care and services provided in a timely and efficient manner according to the agency’s policies and procedure.
- Performs other duties as assigned.
QUALIFICATIONS:
- Currently certified as a Home Health Aide and/or Nurse's Aide in Oklahoma.
- Current CPR/First Aid certification through the American Heart Association.
- Must be able to function independently in the home care setting with minimal direct supervision.
- Current driver's license and proof of automobile insurance in this state.
