Exciting opportunity for a Licensed Social Worker or Social Worker Designee!

Job Summary: Responsible for coordinating and managing the completion of the Minimum Data Set (MDS) assessments in compliance with regulatory requirements. This role involves ensuring accurate and timely documentation, collaborating with the interdisciplinary team, and supporting the development of individualized care plans.

Key Responsibilities:

• Coordinate the completion of MDS assessments.
• Develop and implement care plans based on MDS assessments and resident needs.
• Monitor and review care plans to ensure they reflect the current status and needs of residents.
• Prepare and submit MDS-related reports and documentation as required by regulatory agencies.
• Participate in care plan meetings, resident assessments, and other interdisciplinary team activities.

Qualifications:

• Bachelor’s Degree in Social Work or other field respectively.
• Previous experience in long-term care and MDS coordination preferred.
• Knowledge of MDS assessments, care planning, and regulatory requirements.
• Excellent organizational, communication, and analytical skills.
• Ability to work independently and as part of an interdisciplinary team.
• Experience with electronic health records (EHR)
