Experienced APP (3+ years) ~ Transitional Care Clinic ~ Just North of Orlando/30 Minutes from the Beach - AdventHealth Central Florida

AdventHealth Medical Group is looking for an experienced APP (APRN or PA) to oversee the West Volusia County Transitional Care Clinics.

We are seeking an APP, with a team player mentality, and a dedication to continuing our tradition of excellence in both clinical practice and professionalism, while maintaining AdventHealths mission, vision, and values.

The position will require the ability to manage complex, high risk, high frequency patients with the goal of reducing inpatient readmissions. The APP will collaborate with an on-site pharmacist, respiratory therapist, social worker (community care), and diabetic educator, as needed.

The opportunity has a 4- (10) Hour weekday schedule, with clinic locations in close proximity to our AdventHealth Deland and AdventHealth Fish Memorial campuses.

Requirements of the position: 

• 3+ Years experience
• Board Certification: Board Certification: Physician Assistant or Nurse Practitioner
• Active FL medical license (or ability to secure active, unrestricted licensure)
• DEA License required (or ability to obtain)
• Strong communication skills

In 1996, the Adventist Health System purchased Volusia Medical Center and in 1998, renamed the facility AdventHealth Fish Memorial to honor the hospitals benefactor, Judge Bert Fish.
