Job Description:
Under the direct supervision of a professional nurse, participates in the nursing process and provides and directs the nursing care during scheduled shift to ensure that the needs are met for each person living in the neighborhood. Within your scope of practice, you will be in charge of the day-to-day clinical aspects of care for each person in the neighborhood family. As a neighborhood team member, you will contribute to the development and implementation of care strategies for each person and help identify what they need to achieve and maintain their quality of life and care, providing those needs in a holistic approach. Respect the decisions of each person, accepting their individuality, abilities and choices will be the focus on which care strategies will be developed and delivered as well as being based on person centered choices. Provides medication and treatments, direct nursing care according to their needs and will be responsible for managing their quality of care and quality of life issues. Assists with the completion of the MDS process when needed and to develop, change and implement an acceptable plan of care within the scope of a practical nurse. You must posses and utilize critical thinking skills to perform the duties competently within the scope of practice of your licensure. You must be able to function as part of a team, conduct yourself in a professional manner and communicate effectively to others. You will be responsible for maintaining a positive attitude within the team, for problem solving and conflict resolution of a wide variety of situations. As a Licensed Practical Nurse you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your assigned duties within your scope of practice. Reports to the Director of Nurses and to the Extended Care Administrator for Administrative report. The omission of specific statements of duties does not exclude them from the position if the work is similar, is a logical assignment to the position, within your scope of practice or a reasonable request.
